It comes as Victorian health authoritiesinvestigate a new case of COVID-19 in the community after a recently returned traveller tested positive to the virus in Melbourne on Tuesday morning.

He had completed his hotel quarantine in South Australia after returning to the country on April 19 from India,via the Maldives and Singapore.

The man,aged in his 30s,tested negative to coronavirus on days one,five,nine and 13 of his stay.

He attended a number of venues in Melbourne’s north and CBD while potentially infectious.

The declaration forms are available on the Service NSW website.
