“This incident shows how important exhaustive expert verifications are to prevent a lift from going wrong and how significant the consequences could be,” the note said.

The Sinbad platform is within the jurisdiction of the WA government as it is near the coast.

Department of Mines,Industry Regulation and Safety deputy director general Ian Munns said the investigation into the incident at Sinbad was ongoing.

“DMIRS is aware the incident exposed workers at the site to serious risks,” Mr Munns said.

DMIRS has prohibited Santos from performing similar cutting and lifting operations planned for the Campbell platform until the investigation into the Sinbad incident has been completed.

The Sinbad and Campbell platforms were installed in 1993 and have been inactive for 15 years since production ceased in 2006.

Santos plans to decommission up to 17 structures in the area over the next five years.

Santos did not respond to questions. Fugro was approached for comment.
